All Clients must fill out the 2 page First time client health history form and sign in order to be treated.

Client can print, fill out form prior to the massage session, and therapist will review upon arrival or therapist can provide a copy to fill out at the time of arrival to the appointment. Massage time does not star until client is on table for massage.
